As a consumer of Sunflower Developmental Service, we collect your Personal Health Information directly from you or the person(s) acting on your behalf or from which you have given consent.The information collected, may include, but is not limited to the following identifying details:

• Name, date of birth, email address, address and phone number 

• Physical and mental health history (Assessments/ Diagnosis)

• Records of your visits to Sunflower Developmental Services and the services you received  

• Personal Health Information about you from other sources, including your family physician(s) as permitted by law or if we have obtained your consent 

 

How we store your information?

We store your Personal Information digitally in a centralized database system. This tool ensures that your information is accurate and safely stored. Other storage may consist of doubled locked cabinets.

How is the information used?

We use and share your Personal Information with external services and health care providers as permitted by law, as well as to consult with other care providers affiliated with, Sunflower Developmental Services to support the services you are receiving.

 

Depending on the situation, your personal information may also be used to receive payments for services and for teaching opportunities.

Your rights?

Access 
Your Personal Information belongs to you & you have the right to request access to your information and a list of services or providers who received your information. Even after discharge, your personal information remains stored, for 7 years.

 

Corrections/ Updates
You can request to correct or make updates, to ensure your information is, complete and accurate.

Confidentiality 
We have an obligation to protect your information from, being duplicated, copied, going missing, being stolen, and others sharing or misusing it. You can also withdraw your consent, and we will stop using and sharing your information.

Cookies and Website Data Collection

Similar to many websites, our website uses and collects digital “cookies” to gather general trends, data concerning traffic on the website for statistical analysis. A “cookie” is a small piece of computer code that collects your data while you browse. Cookies by themselves do not tell us your email address or otherwise identify you personally. We also track general information on visits to our website to better understand and improve the information & services users are accessing. This information may include numbers of visitors to the site and specific pages viewed, referring websites, length of visit on the site etc. This information is used to improve the content and experience of our website. Sunflower Developmental Services may use third parties to collect data from our website anonymously for marketing & advertisement purposes. Website users will not be personally identified through this data collection, and Sunflower Developmental Services does not see any data collected or contact information on an individual level. These third parties may include but are not limited to Google Analytics, Facebook, Instagram or Twitter.
